Aidbox Forms

This SMART on FHIR app integrates with Aidbox Forms to seamlessly administer questionnaires within your EHR or patient portal. It supports both practitioner (EHR launch) and patient (patient portal launch) workflows. Questionnaires are dynamically fetched from the Aidbox Forms Public Builder, allowing you to leverage pre-built forms. Completed questionnaires are saved as FHIR QuestionnaireResponses within your FHIR server, ensuring data consistency and interoperability.
