SMART - PreciseHBR calculator

A PreciseHBR score calculator APP which support the use of SMART on FHIR and CDS hooks. Friendly user interface, fast, and easy to use. Support quick feedback and show important information needed to calculate the risk.

1. SMART on FHIR Launch: A clinician can launch the app directly from a patient'schart in the EHR. The app securely authenticates using OAuth 2.0 and gainsaccess to the necessary patient data.

2. Automated Data Retrieval: It automatically queries the hospital's FHIR server to fetch the required patient data, including:

* Patient demographics (age, gender)

* Lab results (hemoglobin, creatinine, eGFR, white blood cell count)

* Active conditions and diagnoses

* Current medications

3. Risk Calculation & Display: The application processes the retrieved data through its risk-scoring engine and presents a clear, intuitive interface showing:

* The final calculated risk score.

* A detailed breakdown of the factors contributing to the score.

* Clinical guidance and recommendations based on the risk level.

4. CDS Hooks Integration: The app also functions as a CDS Hooks service. Thisallows it to proactively provide alerts and "cards" with risk informationdirectly within the EHR workflow, for example, when a clinician is viewing patient's chart (patient-view hook) or prescribing certain medications (medication-prescribe hook).
